Thank you for your interest in our Eureka After School Club in The Palmas Academy . Below you will find the registration form. 

IMPORTANT NOTE: 

  1. If the quorum is not reached, all registered participants will be refunded whatever payments have been made to their original method of payment.

  2. A processing fee of 3.9% is automatically applied to payments made with debit/credit cards.

  3. Payments can be made in full or in a 2 or 3 month payment plan.  Payment plans incur in an additional $20 fee for 2 months or $30 for 3 months.
